- Verification Form for Housing Accommodations: Student must contact the DSS office to request a copy of this form by emailing Disability Support Services at DSS at DSS@ctcd.edu or calling the DSS Coordinator at 254-526-1863. Students are REQUIRED to have a licensed professional who has been treating the student's disability complete this form, and it must be updated YEARLY for DSS. (This form must be attached to the Student Intake and Request for Housing Accommodation e-Forms)
- Student Intake E-form: students requesting housing accommodations are required to complete and submit the student intake e-form
- Request for Housing Accommodation E-Form: must be completed every semester or term the student wishes to request housing accommodations
These forms must be submitted to Disability Support Services before the request for housing accommodation can be reviewed.
*Morton Hall Housing Application: students are required to complete the housing application with Morton Hall, which is a separate process. Questions should be directed to the Residence Hall Manager.
