Student must contact DSS to request the TSI and/or HESI Accommodation Request Form by emailing DSS at DSS@ctcd.edu or calling our DSS Coordinator at 254-526-1863.
Students must also submit the following required documentation along with the Accommodation Request Form (insufficient documentation will not be accepted by DSS and student will be asked to provide additional documentation).
- Documentation on an official letterhead by a qualified professional whose license/credentials are appropriate to diagnose the disability - must have signature and credentials of the health care professional.
- A diagnosis of the disability or disabilities for which testing accommodations are being requested, summary of tests (along with test results) that were used to determine the diagnosis, and explanation of functional limitations due to the disability must be included in the documentation.
- Recommended test accommodations determined by the evaluating professional that are specifically related to the disability and are reasonable within the context of the TSI or HESI and rationale for the test accommodation must be included in the documentation.
After submission and review has been completed by DSS, you will receive a response via email on any approval or denial of accommodations before the TSI/HESI test date. Submitting the request and providing documentation does not guarantee approval of accommodations.
Students are responsible for contacting CTC's Testing Center directly to schedule their test appointment. Information on CTC's Testing Center can be found at: https://www.ctcd.edu/students/current-ctc-students/testing-services/
