In order to receive accommodations, students must request accommodations every term/semester by completing the Accommodation Request eForm in eTrieve. Accommodation forms must be updated each semester or when students add new classes they do not already have accommodations for. If a student does not have a current Accommodation
Letter from the DSS office for the course, the instructor will not provide any accommodations until the student has submitted the Accommodation Request eForm to DSS for accommodation and has been approved for accommodations by the DSS Coordinator. It is the student's responsibility to request accommodations from the DSS office. The DSS office does not send out accommodation reminders. DSS students may request academic advising from the DSS Coordinator – student responsible for scheduling an advising appointment with the coordinator.
