- Complete/submit the required Student Intake e-Form for classroom accommodations at https://etcentral.ctcd.edu/ and attach required medical documentation to the Student Intake e-Form
- After intake e-Form and medical documentation has been submitted, the DSS Coordinator will review and contact the student. If submitted documentation is deemed insufficient by DSS, student will be asked to submit the required missing information.
- Students must schedule intake meeting with DSS Coordinator before final accommodations will be approved.
- Student must submit the Online Accommodation Request Form EVERY semester for accommodation requests ONLY after the intake process has been completed and documentation has been approved by DSS
